Work Assessment Form

To Be Completed by the Treating Physician

Temporary Rx Card

If you need to fill a prescription, you can use the First Fill Card. This card is a temporary solution until your physical prescription card arrives in the mail. You should expect to receive the physical card within 7-10 business days. The First Fill Card can be used at major pharmacy chains for prescriptions related to your workers' compensation (WC) injury. Please note, this card is only valid for prescriptions directly related to your WC injury.

Return to Work Form

You need to have a Work Assessment Form completed by your employer. Please ensure that your provider completes a Work Assessment Form to outline your recovery process and work status. After each appointment, kindly send the completed form to both your employer and examiner. Our goal is to help you return to work as soon as possible.

Diagnostic Testing

If your provider prescribes any diagnostic testing related to your work-related injury, please contact your examiner directly to schedule the appointment.

Work-Related Injuries

When accessing care for work-related injuries, most occupational clinics and urgent care centers accept workers' compensation. If you need assistance accessing or scheduling an appointment with a specialist that accepts workers' compensation, please reach out to your examiner. In the event of an emergency, please go to your nearest hospital
